"Hubsan support form"

Hello everyone. Anybody know where I can find "Hubsan support form". I have to fill but I couldn't see in Hubsan website.
Never heard of it. Only form of communication is by email and then wait... and wait... and wait.
I am lucky if I get a response to any email withing a week and 1/2.
